Paroxysmal SVT: Atrioventricular Nodal Reentrant Tachycardia (AVNRT)

AVNRT is a reentrant arrhythmia that circles within the AV node or peri-AV nodal tissue. This arrhythmia is thought to occur when there are two extensions on the AV node (often referred to as “dual AV node pathways” or “dual AV node physiology”). The two AV nodal extensions generally have distinct conduction velocities and are referred to as the “fast pathway” and “slow pathway.” The reentrant loop usually travels down the slow pathway and then turns up the fast pathway to complete the loop. The electrical signal branches off to the ventricle at the same time that the retrograde fast pathway is activated. Since the atrium and ventricle are activated simultaneously, the P waves and QRS complexes overlap. Thus, the retrograde P waves are inverted and buried within or “married” to the back of the preceding QRS complex with no separation between the two.

PSVT recognition clues:Classically presents as palpitations that start and stop abruptly.
